Below is the application for the HCAP and Pomerene Hospital Charity Care programs. Please complete all items as they pertained to your financial situation 3 months prior to date of service. In addition to the application, you must include all of the following documentation in order for your application to be processed. Holmes County residents will be screened for the Charity Care Program as well.
